The Primary Care Learning Disability Project has been established by Hastings and Rother CCG in order to help reduce health inequalities amongst people with learning disabilities in the local area. To work towards this we are promoting the Learning Disability Annual Health Check for people over 14 years and the Health Action Plan that is created from the health check. We also recommend that people are on their GP practice’s learning disability register so all staff at the surgery will be aware that reasonable adjustments may need to be made. It is also advisable that people consent to sharing their information on their Enhanced Summary Care Record so that when accessing any kind of health service, staff will know about the person. A ‘This is Me Care Passport’ will also provide staff with vital knowledge to ensure a person with learning disabilities has quality, personalised support.
